Visit Your nearest Urban health center or Civil Hospital
Required Document:-
👉Income certificate
👉Address proof(Aadhar card,Light Bill,Licence,Passport etc.)
👉Finger Print of 5 family members
By successfully completed your document procedure you will get this card on same day.
